    Do not use this form to report events presenting an immediate threat to life or property. Reports submitted through this form may not receive an immediate response.

    This form is designed to provide students, employees, faculty, vendors, visitors, or others with an online method to report specific information related to an alleged incident(s) where Middlesex College’s Code of Student Conduct was violated.

    You are not required to complete the entire form in order for your report to be submitted. Anonymous reports are permitted but may limit the College’s ability to investigate and respond to complaints. 

    Middlesex College will use the information provided to begin an investigation, which may include contacting the complainant, respondent, and/or any potential witnesses. The College’s ability to respond is enhanced based on the quantity and quality of the information provided.

    • Academic Dishonesty Procedure Recommendation 
    • If you are a faculty member and have not yet met with the student regarding the alleged academic dishonesty, please follow the procedure outlined below before proceeding with the report.

      1. Request a Meeting with the Student: Upon discovery of an alleged violation of Academic Integrity by a student, the faculty member shall, within seven (7) working days, contact the student and request to meet with the student to understand the scope and intentionality of the issue in question. In addition, as soon as the alleged violation of Academic Integrity occurs, the faculty member shall notify the Department Chair so that the Chair is aware of the situation. 

        If the student responds and agrees to the meeting, Step Two of the procedure will commence. If the student does not respond, is unwilling to meet with the instructor, or fails to show up at the meeting, the student will not be able to register for another course until the issue is addressed, and the faculty member should continue to fill out and submit this form. 

      2. Meet with the Student: When meeting with the student, the faculty member should attempt to discern
        • if a violation of academic integrity was committed, and, if so,
        • whether or not the incident rises to the level of a reportable offense. 
            
          To make these determinations, the faculty member should consider what the student knew (or should have known) at the time of the alleged violation. Was the violation described in the course syllabus? Was it discussed earlier in the semester, perhaps during the first week of class? If appropriate, was instruction in the area of the alleged violation provided earlier in the semester (e.g., plagiarism, citing sources, etc.)? Are there any other reasonable explanations for the incident? Was there a violation of department or program policy? 
            
          Further, if the faculty member determines that a violation has been committed, they should also consider if the situation can be addressed within the context of the course, or if it should be considered a reportable offense. Addressing the issue within the course might be used in situations that deal with minor or “low stakes” assignments; involve valid extenuating circumstances (e.g., extreme duress due to death or illness in the family, etc.); or present a “teachable moment” in which the faculty member decides to work with the student. Working with the student might include the following options or some variation/combination thereof: 
          giving a warning along with an explanation of the offense, an activity to address the offense, and the potential penalties of a repeat offense; 
          giving an “F” for the assignment with the possibility of re-submitting the assignment for full or partial credit; 
          giving an alternate assignment for full or partial credit. 
            

          Incidents that might rise to the level of a reportable offense include those that involve major or “high stakes” assignments, repeated and/or escalating incidents, intentionality, collusion, and the like. 

      3. Report the Incident: After meeting with the student, if the faculty member determines that academic dishonesty occurred and that it rises to the level of a reportable offense, the faculty member should notify the Department Chair and then submit the Academic Integrity form to the Office of Student Engagement.
