Do not use this form to report events presenting an immediate threat to life or property. Reports submitted through this form may not receive an immediate response.

This form is designed to provide students, employees, faculty, vendors, visitors, or others with an online method to report specific information related to an alleged incident(s) where Middlesex College’s Student Code of Conduct was violated.

You are not required to complete the entire form in order for your report to be submitted. Anonymous reports are permitted but may limit the College’s ability to investigate and respond to complaints.

Middlesex College will use the information provided to begin an investigation, which may include contacting the complainant, respondent, and/or any potential witnesses. The College’s ability to respond is enhanced based on the quantity and quality of the information provided.
